Adult Care 

A successful transition to adult-centered care ensures that youth with epilepsy get a strong start towards a healthy life.

Growing up isn't easy! Our Transition Together program can make this process easier for young adults ages 14 to 22.   

​

What is transition? 

Health care transition is the process of changing from a pediatric to an adult model of health care. The goal of transition is to optimize health and assist your child in reaching their full potential. It involves change from a “pediatric” model of care where parents make most decisions to an “adult” model of care where youth take full responsibility for decision-making. This enables your child to become capable of managing their health just as they would manage all other decisions and aspects of their life.

Now you or your child can be matched with an experienced mentor who has been screened and trained to assist with common obstacles that come with transition. Mentors help youth...

  • self-advocate in clinical environments

  • become more aware of mental and social health self-care

  • connect with their community

  • form life-long friendships!
